To do this, I set the window borders to 1 pixel, created the titlebar at 800x22 pixels, with a very fine gradient on the left edge that goes from the window frame color into the titlebar red gradient colors. (the inactive window one just gets progressively darker from the right edge to 60 pixels in, then stays a darker red to the grey fade on the left).I then used the titlebar background as the background for my 22x22 buttons (close, maximize, roll, etc) and put the top edge of the button area at the very top edge of the 22x22 button graphic (without a top border on the visible button graphic - the 1 pixel window border becomes the top border of the button area). The button outlines are created using a 3 point gradient that starts (at the top) with the color of the window frame, goes to a slightly red tint in the middle, and then to black for the bottom edge.
The background (and all the rest for that matter) was made using Embellish with some editting in PMView (Cropping the buttons from one continuous button bar, etc). I just laid out shapes for the icon areas, added gradient effects and a few borders, starting with the red one faded in, and finishing with the black one being non-fade... and then laid out my icons to fit. Still havent finalized an icon scheme though (thinking of going all to the grey icons, but I have a lot more to make and still need to reduce the colors... the grey icons are from an icon pack posted on Hobbes, with some modifications I made to various of them).
